She is survived by: her children, Alphonse, Audrey and Sabastian; her parents, Sherri Hayward and Michael O'Hara; and her siblings, Natasha, Jauna, Aurora and Ryan. She is also survived by her many nieces and nephews, extended family and close friends.
In lieu of flowers donations made to any local children's charities, mental health, or addiction awareness programs would be appreciated by the family.
