He was predeceased by: his father John Smythe; and his siblings, Tina Devost and Jamie Smythe. He is survived by: his children, Quinn, Raeanna, James and Leland; his mother Yvonne Chappelle; and his siblings, Joe Smythe (Nicole), Cory Chappelle (Danielle), Dylan, Hollie (Nick), Herb Jr. Jolene Hall (Randy), Brent Lee Chappelle, John Smythe Jr., Paul (Becky), Rick Loxton, Bonnie, Cheryl (Lyle) and Doug Smythe. He is also survived by many aunts, uncles, nieces, nephews, cousins and friends.

Sympathy may be expressed through donations to the Ontario Heart & Stroke Foundation.
