He was predeceased by: his parents, Jacob Cornelius and Adriana Bax (Verhoog); his siblings, Jim, Joanne Vanderploeg and Mary Hummelen; his brothers-in-law, Bert Hummelen, Bill VanderPloeg, Albert Herfst and Adrian Wielhouwer; his niece Tracy Zwiers; and his nephew Paul Herfst.
He is survived by: his spouse Barbara Bax; his children, Ron Bax, Anita Sukenik (Jim), Terry Bax (Pamela) and Dave Bax (Kendra Lowry); his siblings, Margaret Herfst, Ruth Bax, Gertie Wielhouwer, Jean VanHelden (Pete), Clarence Bax (Lorraine), Bernie Bax (Tina), Hank Bax (Margriet) and Herb Bax (Frits Bax); and his siblings-in-law, Lucy Wright (Bun) and Siebe Bruinsma (Kathy). He is also survived by nieces and nephews.
