He was predeceased by: his sister Dawn Golba; and his parents, Donald Polkinghorne and Dorothy Polkinghorne. He is survived by: his wife Anne Phillips; his grandchildren, Ethan, Jacob, Patrick Morden, Vanessa Field, Jordan, Dylan, Maddax, Robert Morden and Adrienne Heikkila; his brother Barry Polkinghorne; and his nieces, Natalie and Lisa. He is also survived by relatives and friends.
Donations in his memory can be made to the Sudbury Game and Fish Association, (Arrangements entrusted to the Jackson & Barnard Funeral home), We request no flowers due to family medical conditions.
