He was predeceased by: his parents, Malcolm MacMillan and Louise MacMillan; his parents-in-law, George and Olga Stewart; and his sister-in-law Gill Hearn.

He is survived by: his friends, Tom Chandler, Eric Bodkin and Reid Mason; his significant other Marnie; his close friend Bill Esarik; his children, Joanna Martins (Paulo), Janet MacMillan (Bruno) and Malcolm MacMillan (Laura); his grandchildren, Sofia, Callum, Isabella Martins, Lua, Selah Sousa, Cameron and Ethan MacMillan; and his sister Mary Lou O'Donnell (Randy). He is also survived by numerous nieces and nephews, his brother.

In lieu of flowers, Jack would have appreciated donations to the Shriners Hospital for Children and the Thunder Bay Regional Health Sciences Foundation – Northern Cancer Fund or The Thunder Bay Humane Society.
