He was predeceased by: his parents, Leon and Maria; his granddaughter Kara; and his brothers, Rudolf (Irene), Ted and Mitch.

He is survived by: his wife Teresa Puklicz (Szymanska); his son Andrew Puklicz (Cheryl) of London; his grandchildren, Kory (Halie), MacKenzie (Cameron) and Barbara (Darrell Smith) of Tillsonburg; his great grandchildren, Craig (Meghan), Kyle (Stephanie) and Connor (Selina); his grandson Adam Puklicz of Windsor; and his great grandchildren, Aaron, Landon, Ethan, Aubrey, Luca, Celia, Grace, Eleanor, Russell and Lorelai. He is also survived by nieces, nephews and cousins.
