He was predeceased by: his wife Olga; and his siblings, Annie Slawski, John Kotva, Stan Kotva and Walter Kotva.

He is survived by: his significant other Patricia Smith; his sons, Rick (Bonnie), Gary (Jean) and Terry (Brenda); his grandchildren, Kareen, Breanne (Chris), Krista (Jenn), Taylor, Robert, Jonathon and Alexandra; his great grandchildren, Eden, Reed and Sloane; his sister Julie Doherty; and his sister-in-law Edie Kotva. He is also survived by numerous nieces, nephews.
