He is survived by: his wife Halya Polatajko (Helen); his son Christopher Kozub; his daughter-in-law Allyson Marta; his grandchildren, Hannah Kozub (Hanusya) and Lev Kozub (Levchyk); his sister-in-law Slava Komichak; his niece Olesya Komichak; and his extended family Ohio. He is also survived by the many other relatives and friends.
