He is survived by: his children, Theresa, Leonard, Elizabeth Kolotyliuk, Cathy (Scott Carson) and Kate Collins; his grandchildren, Nicole (Kenny), Tom, Kurtis (Cindy), Zachary, Joseph, Andrew, Caitlin (Neal), Mitchell (Eddie), Ashley (Don), Kristina (Dan), Evelyn, Harlan, Jaxon, Hayden, Brooks, August, Amelia, Samantha, Tristin, Melissa, Wesley and Miles; his niece Dee; his nephews, Gerry and John; and his significant other Mary Lou Knight.
