He was predeceased by: his parents, William and Catherine. He is survived by: his spouse Tammy; his children, Tabitha (Joe) and Tory; and his siblings, Lee Ann Forsyth-Sells (John), William (Janette) and Janice Forsyth (Dan Hallett). He is also survived by many nieces, nephews and lifelong friends.
