He was predeceased by: his grandparents, Gennaro Mastromattei and Carl. He is survived by: his parents, Kelly and Andy Mastromattei; his grandparents, Maria Mastromattei and Carol; his cousins, Melissa, Bianca, Brandon, Giulia, Joshua, Brooke, C. J. and Grayson; and his pet Molly.
As an expression of sympathy and to honour Jake's life, memorial donations may be made to the Windsor and Essex County Humane Society.
