He is survived by: his parents, Evelyn Ballantyne and Michael Ballantyne; his child Jaden Cook (Angel J.); his siblings, Stacey Dorion (Travis), Laurie Ballantyne (Greg), Claudine Ballantyne (Thomas), Clayton Ballantyne (Diandra), Abel Ballantyne and Zane Ballantyne; his grandchildren, Ellie Ballantyne, Zoey Cook, Arielle Lexi and Floyd Royce Ballantyne (Boy); his goddaughters, Natasha Ballantyne and Tara Ballantyne; his aunts and uncles, Charlie Sewap, Simon Cook (Norma), John Cook (Loret), Samson Cook (Darlene), Douglas Cook (Debra), Oliver Cook, Fred Thunderchild (Alvina), Sophie Michel (Wilson), Lisa Trudue, Melanie Cook, Shirley Cook (Dennis) and Ardelle (Martin); his grandmother Margaret Ballantyne; and his close friends, George and Kathy McCallum.
