He was predeceased by: his parents, Melba Flach and Ferdinand Flach; and his brothers-in-law, Norman Paproski and Ernest Uhryn. He is survived by: his wife Pat Flach; his children, Deanna (Ashley), Sadie, Dawson and David Flach; his stepchild Chris (Shannon); his children, Kasey, Carter, Ryan Cederstrand (Alyssa), Kerri (Russ), Rowan and Lucas; and his siblings, Marlene Paproski, Donna Flach and Ron Flach (Deb). He is also survived by many nieces, nephews, extended family, and friends.

In lieu of flowers, donations in memory of Jack can be made to the Saskatchewan Cancer Society.
