He was loved and cherished by many people including: his parents, Dale Holstein and Sherry Holstein; his sons, Damion and Riley; his grandparents, Casey, Dorothy Weenk and Benjamin Holstein; his aunt Ramona; his siblings, Dominique Hayden (Tom), Brendon (Carol) and Bobby; his nephews, Ethan and Pacey; his niece Serena; his close friend Natalie Smith; his grandmother Lorraine; Devon, Tristan and Victoria. He was also cherished by numerous aunts, uncles, and cousins. and his lifelong friends.

In lieu of flowers, memorial donations may be made in David's memory to a trust fund set up for his boys.
