She was predeceased by: her parents, Leonard Robert and Comelia Abbott (McAndie); and her parents-in-law, Mary Polachek and Joseph Polachek. She is survived by: her husband David Polachek; her daughters, Angelina Kardynal (Larry), Prudence Achtymichuk (Chad) and Joelle Smith (Mark); her grandchildren, Jagger, Willow Kardynal, Ewan Achtymichuk (Maureen), Jude, Georgina Achtymichuk, Hudson and Emily Smith; her great grandchild Kingston; her brothers, Lennie Abbott (Elaine), Ian Abbott (Joanne) and Bobby Abbott (Sophie); and her brothers-in-law, Kenny Polachek (Beverly), Ronnie Polachek (Jeanette), Keith Polachek (Deb Mosewich) and Gordon Polachek. She is also survived by many cousins, nieces and nephews.

Memorial donations in memory of Patricia Polachek may be made to the Cancer Foundation of Saskatchewan as gifts of remembrance.
