He was predeceased by: his parents, William Cook and Tessie Mays Cook; and his siblings, Millard Cook, Grover Cook, Homer Cook and Helen Sizemore.

He is survived by: his brother Cecil Cook (Joy); his children, David Cook (Lora), Buddy Cook (Lesia), Wayne Cook, Earl Cook, Scott Cook (Sarah) and Becky Hinkle (Joey); and his grandchildren, Jeremy Cook, Colby Hinkle, Karlie Hinkle, Katie Grace Cook, Byram Cook, Matte Cook and Sayde Cook. He is also survived by nieces, nephews and friends.
