He is survived by: his children, Jack Fields Jr. (Salina), Ronald Fields (Rochelle), Dana Stinson (Fran), Shelia Fields Pritchett (Antony), Gwendolyn Fields and Natasha Jackson; his siblings, Charles Weaver (Ethel) of Montgomery, Alabama., Carrie Ann Fields of Birmingham, AL., Annie Tate (Edward) of Birmingham, AL. and Terri Maull (Chris) of Birmingham, AL.; his aunt Mary McDaniel of Birmingham, Alabama.; his grandchild Thirty; and his niece Shirley Reynolds (BooBoo).
