He was predeceased by: his father Thomas Jefferson; his wife Katherine Inez Johnson Warren; and his daughters, Cynthia Dale Warren Gray and Anita Faye Warren O'Neal. He is survived by: his grandchildren, Greg Alan O'Neal (Myra Pate), Amanda O'Neal Whitfield (Matthew Dale) and Justin Cody (Samantha). He is also survived by six great-grandchildren.
In lieu of flowers, the family suggest memorial donations to the Marked for Life Ministries, Hope Center, 2141 Gault Avenue North, Fort Payne, AL 35967.
