She was predeceased by: her parents, Leonard Bothwell and Willie Mae Bothwell; her husband Jerry Polk; and her siblings, Hazel Looney, Betty Bothwell, Henry Bothwell, Leon Bothwell and Oscar Bothwell.
She is survived by: her children, Frederick Polk (Tammye), Dimica Polk and Symone Campbell (Justin); her grandchildren, Jerry Polk, Khloe Campbell and Justin Campbell Jr.; her siblings, Mary Evans, Dorothy Phillips (Curtis), Wanda Looney, Leonard Bothwell Sr., Clarence Bothwell (Bridget) and Edward Bothwell; her siblings-in-law, James Looney, Nathan Polk, Sidney Polk, Beverly Bothwell, Florence Bothwell, Shirley Beasley and Phyllis Polk; her close friend Deborah Montgomery; and her children, Dehaun Bothwell, Thomas Bothwell, Tommy Bothwell and Teresa Turner. She is also survived by aunts, uncles, nieces, nephews, cousins and many dear friends.

Carolyn was dearly loved in the community and was best known for her charity.
