He was predeceased by: his parents, William Cook and Ollie Cook. He is survived by: his sons, Bradley Cook (Jessica), Brandon Cook (Nicole) and Tyler Cook (Cayla); his grandchildren, Anna Grace, Brodie, Brentlee, Hadley and Weston Cook; and his siblings, James Cook (Elaine), Glenda Dison (George) and Wanda Cook. He is also survived by nieces and nephews.
In lieu of flowers, memorials may be made to the Children's Hospital of Birmingham.
