He was predeceased by: his parents, Clovis Churchwell and Lennis Churchwell; his wives, Ruth and Christine; his son Terry; and his siblings, Faye Phillips, Jerry and Larry Churchwell.
He is survived by: his children, Anthony, Stephen, Gregory Ray (Pam), Scotty and Debra; and his siblings-in-law, Faye Churchwell and Charles Staggs (Charlene). He is also survived by grandchildren; great-grandchildren; and several nieces and nephews.
