He was predeceased by: his parents, James K. Polk Sr. and Susan Krceck Polk; his brother James K. Polk Jr.; his uncles, Woodrow Polk and Arle Polk; and his aunt Jenny Polk.

He is survived by: his great-nephew Nathan Fauchman.

In lieu of flowers the family request donations be made to the Trustmark Bank in Montevallo, Alabama in Memory of Tommy Polk to assist the family with medical and funeral expenses.
