He was predeceased by: his parents, Clark Cook and Helen June Cook. He is survived by: his wife Linda Cook; his daughter Alison Golden; his grandchildren, Camryn Golden and Collins Golden; and his siblings, Anne Graben, Joe Cook and Clark Cook, Jr.. He is also survived by nieces and nephews.
