He is survived by: his wife Jo Ann Huffman Cook; his son Charlie Thomas Cook, III (Kellie); his grandchildren, William Thomas Cook (Cheyenne) and Elizabeth Gray Cook (Tristan); his great grandchildren, Ethan, Jaishonna, Emery, Keira, Wylin and Sloane; and his siblings, Sara Elizabeth Cook Semmes (Thomas) and Edward Cook (Julia). He is also survived by loving nephews and nieces.

In lieu of flowers, donations may be made to Hospice of West Alabama in Mr. Cook's honor.
