It is with great sadness that we announce the death of David Mark Kulak (Cedarville, Arkansas), born in Pittsburgh, Pennsylvania, who passed away on August 4, 2019, at the age of 26, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He is survived by : his parents, Judith Kulak of New Kensington, PA and Mark Kulak; his wife Diane; his siblings, Mary Peterson (Shawn) and Erich Kulak (Megan); his stepsisters, Teresa Ray (Eric) and Heather Gilmore; his nephews, Bobby, Liam, Daxsyn and Tristan; and his pet Jazzy.

Donations in lieu of flowers can be made in honor of David to the National Alliance on Mental Illness or Puppies Behind Bars.
