He was predeceased by: his parents, Clearance Otis 'Happy Lewis and Paula Mae Goodman Lewis; his daughter Diana Lynn Lewis Hunt; and his siblings, Robert Lewis, Jack Lewis, Sam Lewis and Barbara Lewis Smith.
He is survived by: his spouse Kelly Blankenship; his sons, Michael Paul Lewis of Forrest City, Weston Ray Lewis of Forrest City and Dustin Albert Lewis of Forrest City; his sister Ida Shelby Cox (Paul) of Nixa; and his grandchildren, Johnathon Hunt and Charlie Nicholas Hunt. He is also survived by several great-grandchildren.
