He was predeceased by: his parents, Herman Churchwell and Henrietta Churchwell; and his grandmother Mamie Goodwin. He is survived by: his son Eric Churchwell; and his siblings, Donna Churchwell, Deborah Churchwell and Herman Churchwell.
