He was predeceased by: his parents, Hugh Cook and Vivian Cook (Smith); and his siblings, Carlton Cook and Linda Cook. He is survived by: his wife Carolyn Cook (Varon); his children, Shawn (Paul) and Tony (Claudette); and his grandchildren, Taylor Cook Boesche (Matthew), Jesse Cook, Faith and Hailey Dumais. He is also survived by many nieces and nephews.
In lieu of flowers, a memorial contribution can be made to Shriners Children's Hospital in Shreveport, Louisiana.
