He was predeceased by: his parents, Chester Izard and Frances Izard; his daughter Robin; and his sister June. He is survived by: his children, Dana and John Jr.; and his wife Carolyn. He is also survived by several grandchildren and great grandchildren.

Due to the COVID-19 pandemic, there will be no memorial service.

John R. Izard Sr. died September 15, 2021 peacefully after a long battle with Alzheimer's.
