It is with deep sorrow that we announce the death of Deonna Lynn Jacobs of Mesa, Arizona, born in Victorville, California, who passed away on January 16, 2021, at the age of 44, leaving to mourn family and friends. You can send your sympathy in the guestbook provided and share it with the family.
She was predeceased by: her father David Bernhardt; and her grandfather Wayne Perry. She is survived by: her mother Susan Perry-Carnahan; her husband Cody Jacobs; her children, Brittany, Dalton and Logan; her stepfather Jim; her brother Jeromy; her parents-in-law, Greg, Susan, Terry and Val; and her grandmother Carole. She is also survived by step-brother, two-half brothers, many aunts, uncles, nieces, nephews, cousins and friends.
An outdoor memorial celebration will be held on January 28th at 2:00 pm at the Valley of the Sun Cemetery, located at 10940 East Chandler Heights Rd, Chandler, Arizona The family requests that in lieu of flowers, those who wish to remember Deonna in a special way may make gifts in her memory to My Hope Bags, empowering women living with breast cancer at https: //http://www.myhopebag.org .
