She is survived by: her parents, Robert Koltiska and Dawn Koltiska; her siblings, Kristine Koltiska and Brandon Koltiska; her niece Myka Koltiska (Jaron Mentock); and her pet Tyler.

In lieu of flowers, the Koltiska family is setting up a memorial fund, more details are to follow.
