He was predeceased by: his parents, Howard and Edith; and his siblings, Dean and Sandy. He is survived by: his wife Edie Curtis; his sister Cindie Poulter (Cook); his former spouse Carrol Marie List; his son Tracy Cook (Misty); his former wife Connie Lynn Dutton; his sons, Shane Cook (Jenny), Michael Todd Sherman, Jaymie Cook (Cassie) and John Sherman (Debbie); and his grandchildren, Jacqui, Ellis, Rebekah, Alina, Elliot, Micah, Mason, Madi of Angel, JJ, Lexi, Dan and Topher. He is also survived by five sons, several grandchildren.
