He was predeceased by: his parent Michael Kozak; and his siblings, William Kozak, Peter Kozak and Lillian Tall. He is survived by: his spouse Genevieve "Jane" Kozak; his parent Anna Tener Kozak; his daughters, Mary Jane Boladz (Henry) of Ansonia and Cynthia Ryall (William) of Ansonia; his brother Franklin Kozak; his grandsons, William Ryall III of Stratford and Christopher Ryall of Shelton; his great grandchild Parker William Ryall; his sibling's child Theresa Smith; and also, Michael D'Alessio (Elaine) of Ansonia and Anthony D'Alessio Jr. (Gayle) of Ansonia.
