Cook Jr. of Gardner, ME, Tanya McAfee (Wayne) of Gardner, ME and Adam H. Cook (Heather) of Naugatuck; his grandchildren, Holly, Lindsay, Emily, Isaac, John, Hailey and Meegan; and his brothers, Edward H. Cook (Jean) of Southington and Ross E. Cook (Carol) of New Hampshire. He is also survived by several nieces and nephews.
