He is survived by : his parents, Joseph S. Formica, Sr. of Middletown and Heidi Fields-DiNino of Old Lyme; his stepfather Dino DiNino of Old Lyme; his siblings, Christopher Dasconio of Boynton Beach, FL, Frank Maruca, Jonathan Formica of Middletown, Brittany Maruca of Middletown, Erin Dasconio of Middletown, Samantha Formica of Middletown, Addison Formica of Middletown and Stephanie Tarquinio of Lebanon; his brother-in-law Dominic Tarquinio of Lebanon; and also, John Raymond, Lisa Raymond of East Hampton, Godson Tarquinio and Nico Tarquinio. He is also survived by wife and son.
