He is survived by: his parents, Helen Perez of New Britain and Juan Perez Sr. of Florida; his grandmother Carmen Casiano; his siblings, Jeovanny Perez, Augustine Perez and Jennilda Perez; his significant other Karolin Tomassian; and also, Puerto Rico and Diamond Perez.

In lieu of flowers, memorial donations may be made to the Go Fund Me Account for Cardiomyopathy.
