She was predeceased by: her parents, Michael Botsko and Mary Botsko (Orlovski); her husband Peter Belejack; and her siblings, Helen Rozdil, John and Michael Botsko.
She is survived by: her daughter Barbara; her brother Robert Botsko (Alice); her sister-in-law Margaret Savchik; her sibling's children, Andrea Konetchy, Jan Moltzan (Herb), Sue Ziman (Doug), Mary Ann Kowal (Al), John Botsko (Jeannie), Ed Botsko (Barb), John Savchik (Brigitte) and Tom Savchik (Peggy); Niko and Andrew DeCaprio.
Contributions in Annie's honor can be made to Saints Peter and Paul Orthodox Church in Meriden or to your local humane society or no-kill animal shelter.
