He was predeceased by: his mother Lucy Butler. He is survived by: his stepdaughter Erin Cotter (Keith); his sons, Malcolm (Marla), Brain (Kate) and Kevin Collum (Kristin Beebe); and the mother of his children Maggie Rawlings Green Bay. He is also survived by six grandchildren.

Donations can be made to the Michael J. Fox Foundation and the St. Norbert College Fund.
