He was predeceased by: his wife Carrie Lee Evans; and his grandson Carl. He is survived by: his sons, Chuck, Tom and Rick; his grandchildren, Chucky, Eric, Juliet and Ricky; his great grandchild Kailee; and his siblings, Bob, Barbara and Jay.

In lieu of flowers, contributions may be made to the funeral home to assist with the funeral expenses.
