He was predeceased by: his parents, Fadie Qualls and Lelia Irene Qualls. He is survived by: his wife Eleanor Qualls (Kaczmar); his daughter Doris Bartkovsky; his son-in-law Ronald Bartkovsky; and his granddaughter Laura Bartkovsky. He is also survived by many nieces and nephews.
A graveside service was held on Thursday, July 17th 2025 at 11:00 AM at the Hickory Grove Cemetery (1825 Idlewild Ave, Green Cove Springs, FL 32043).
