He was predeceased by: his parents, Hubert Church and Ruby Church. He is survived by: his wife Cheryl Dawson Church; and his children, Hank Church (Mindy), Stephani Johnston (Murray), Kimberly Pasienza (Shawn), Dawn Goethe (Jim), Ava and Duke. He is also survived by seven grandchildren; 9 great-grandchild; many siblings, nieces, nephews and friends.
