He is survived by : his wife Rita Nichols of Lake City; his daughters, Jody Koontz (Bill) of Indiana and Janet Cook of Indiana; his stepchildren, Kathy Houk of Lake City and Tommy Houk of Lake City; his sister Shirley of Illinois; his grandchildren, Trey Houk, Trenton Houk, Angela Leap (Brock) and Stephanie Koontz; his great grandchildren, Kollin Leap, Ayden Leap, Meadow Koontz, Sierra Cook, Raven Cook, Lily Koontz and River Koontz; and his great great grandchild Anthony Leap.
