He is survived by : his sister Carlene Leones (Dennis) of Lakeland; his nephews, Peter Costianes and Scott Leones; his niece Deana Ramirez (Juan); his brother Leonard Horak (Arlene); and his nieces and nephews, Jay Horak (Jennifer), Brittney Schwilk (Chase), Kailie Horak and Tyler Horak.
