He was predeceased by : his parents, John Kolodziejczyk and Pauline Kolodziejczyk; his siblings, Edward (Edna), Ann (Stan), Johnny (Wanda), Chester (Jane) and Daniel (Frieda); and his brother-in-law Felix.

He is survived by : his wife Betty Perkins; his children, Kim, R. J. (Carrie) and Brent (Bonnie); his sister Florence; his grandchildren, Patrick (Debbie), Kassie (Travis), Kaylee (Patrick), Aleia, Parris (Tom), Brooks (Travis), Grace and Jack; his great grandchildren, Grant, Gwynie, Lucy, Cade and Carson; and his siblings-in-law, Dorothy and Louie.

Memorial contributions may be made to St. Judes Children's Research Hospital or Hospice of N.W. Ohio.
