He is survived by: his wife Betty Forthman; his daughters, Theresa Forehand and Tammy Goff; his siblings, Mary Ann Hayes and Timothy Forthman; and his sister-in-law Wyona Jones. He is also survived by eight grandchildren and five great grandchildren.
