He is survived by : his parents, John Kukuvka and Mary Kukuvka; his sons, Matthew and Joshua Kukuvka; his siblings, Kathryn Morland-Bleier (Daniel), Juliann Sebastiani (Josh) and Joseph Kukuvka; his nieces and nephews, Jessi, Kristen, Lena, John, Tyler and Katlyn; and his great-niece Adalynn.
