He is survived by: his wife Eloise Rachel Sullivan; his sister Genera Shepard (James) of Chattahoochee, FL; his children, Cynthia Lynn Fernandez (David "Buck") of Wewahitchka, FL and Jonathan Phillip Sullivan (Christina) of Palmetto Bay, FL; and his granddaughters, Cali Grace Fernandez and Leslie Michelle Sullivan.
