He is survived by : his wife Wanda; and his sons, Jason Middlebrook of Winter Haven, Jeff Middlebrook of Lowell and Jon Middlebrook of Winter Haven, Florida.. He is also survived by 9 grandchildren and 3 great grandchildren.
