She is survived by: her husband Thomas Jakubzak; her children, Debra Edwards (Jay), Alan Jakubczak (Debbie), Brian Jakubczak and Cheryl Clark (Dave); her grandchildren, Lauren Mattson (Casey), Taylor Jakubczak (Kaila) and Kevin Jakubczak; her great grandchildren, Brantley Mattson and Lillian Jakubczak; and her siblings, Jerry Houk (Jean), Cliff Houk (Kay), Chuck Houk (Vicki) and Nancy Dwyer.
