He was predeceased by: his parents, Elton Cook and Betty Cook (Ohio). He is survived by: his wife Cynthia Cook Crestview; his brother David Cook; his sons, Stephen Cook and Matthew Cook Port St (Laura); and his grandsons, Christian Cook of Niceville, Samuel Cook and Joshua Cook.
